Recently, you may here a lot of new information about carbohydrate counting, CGMS (method to continuously measure value similar to blood glucose), insulin pump and so forth and may feel rushed or left out, but you do not have to.

Don't want to be told what to do
Needless to say, "the first step in treatment of diabetes is information and knowledge".
Last year, a lady with type 1 diabetes in her early thirties visited our clinic through a referral. She was full of energy. She hates being told by her doctor what to do. "As I have dealt with diabetes on my own, I am going to do so from now on too," she said. She wanted to be a patient at our clinic considering years to come as she was going to get married. Though she says she will and wants to manage everything on her own, her HbA1c level is not good.
"My blood glucose levels tend to fluctuate, and thus my diabetes is rather difficult to treat. Treating my diabetes is different from treating other patients with type 1 diabetes. I have been managing my disease on my own, so I don't want to be told what to do about my insulin dosage.
" These are the details she told me, and she has a point but….

How to control blood glucose well without doing much SMBG
Some type 1 diabetes patients at our clinic with good glucose control do not actually know how much insulin they inject every day and how many times. "I won't know till then how many units of insulin to inject," they say. They do not do SMBG frequently. They probably measure only once or twice a day. "My lifestyle pattern changes everyday, and I won't know what I will be eating until right before a meal most of the time." I think they are completely right. (Therefore, about number of insulin units is written down in their medical record.)
Even so, they do not develop abnormal hyperglycemia, or hypoglycemia during the day or night. They have never failed to get up in the morning and have been fully enjoying their school or work.

Build you own experience on basic knowledge
How are they different from the other lady I mentioned earlier?
First of all, they determine insulin dosage by focusing on what will be happening later (such as the amount of activity and the timing of the next meal) rather than on things at the moment.
Secondly, they understand very well how multiple types of insulin they use lower blood glucose specific to their own case. They know approximately how many units of insulin to inject according to what they eat. They know what to do if they oversleep.
Thirdly, they are able to analyze and identify the reason when a certain units of insulin they thought would be right did not work well.

Have solid grasp of the basics of insulin injection first
When I told the other lady about them, the lady was nothing but surprised at how blood glucose could be controlled in such a manner. "If blood glucose level rises, inject insulin." This was the basics the lady had followed. I assume her doctor has probably told her a number of things in the past, but she did not listen. She must have strongly felt that her blood glucose control would not go well if she followed her doctor's advice.
A lesson that could be learned from this example is that basic knowledge is definitely necessary. Even if you do not know calculus, you need to know addition and subtraction.

Find a way that suits you
Sometimes, you could be bound by too much information.
There is a young patient in his early twenties who has good HbA1c and is not fat.
I do not knot well how he is adjusting dosage of insulin (not necessarily insulin analog but multiple injections of NPH and rapid-acting insulin also). This type of patient does not practice carbohydrate counting though he pays attention to the amount listed in Chart 1 of Food Exchange List. Of course, he does not carry around a calculator. Out of fear that he may be developing hypoglycemia during the night, I had him wear CGMS, but it did not show that he had hypoglycemia in the night. He does not recall that he ever experienced difficulty getting up in the morning due to hypoglycemia. When he comes to see me, I do not get any impression that he is at the mercy of insulin adjustment, what to eat or type 1 diabetes itself.
There is another patient in her twenties who is slightly plump. She came to our clinic through a referral when she was 19 years old and was having a hard time lowering her HbA1c level. However, her HbA1c level finally came down below 7% the other day. She did not want to do SMBG as she felt bound by it if she did. Therefore, she has been treated without doing SMBG for the past three years. Though without doing SMBG, her HbA1c gradually began to come down and finally came below 7%.
It would be the best, of course, if good HbA1c level could be achieved from early on. However, patients initially experience internal struggle when they try to accept having type 1 diabetes or having to inject insulin and so forth. If they are told what to do even if that may be the best way, that would not work because of the struggle between reason and emotion they are going through.
The best way seems to be to find a way that is satisfying, reasonable and suits you.

[Postscript] At ADA Scientific Sessions in 2009
The American Diabetes Association has several awards including Outstanding Educator in Diabetes Award. Professor Edelman at the University of California, San Diego received this award this year. Ever since he has developed type 1 diabetes in his childhood, he has been on insulin. He is a very cheerful person and does not look like a typical professor at all.
He was given the award mainly for his TCOYD (Taking Control of Your Diabetes) activities over many years, which is like a large-scale class for diabetes with patients across states.

何かに縛られている?

 カーボカウント、CGMS(持続的に血糖値に近いものを測定する方法)、インスリンポンプなど、最近、多くの新しい情報をあちこちで耳にされることと思います。しかし、あわてたり、あせったり、する必要はありません。
人から指示されたくない
 「糖尿病の治療の第1は、情報、知識」であることはいうまでもありません。
 昨年でしたか、30代前半の1型糖尿病女性が紹介されて初診されました。とても元気のよい方で、はつらつとしておられました。とにかく医者に指示してほしくない「自分でこれまでやってきたからこれからも自分でやっていく」というわけです。こちらに紹介されたのは、これから結婚するので、今後のこともあり、受診先をこちらにしたいという理由でした。自分で全部する、したいというわりには、HbA1c値はよろしくありません。
 これまでのお話しをよくお聞きしますと、自分の場合「血糖が上下しやすく、むずかしい糖尿病なのだ、他の1型糖尿病の方の治療とは違うのだ」というわけです。だから、「自分で工夫してなんとかやっているからインスリン注射量など、さわってほしくない」というわけです。
 お話しはもっともなことなのですが・・・・・・
SMBGもあまりせずどうやってうまくやっているのか
 当方に来院されている血糖コントロールの上手な1型糖尿病の方など、毎日どれくらいのインスリン注射量を何回注射しているのか、ホントはあまり知りません。「その場に遭遇しないと、何単位注射するかわからない」ともいわれます。かといって頻回にSMBG(血糖自己測定)しているわけではありません。測定しても1日に1、2回くらいか? 「毎日の生活パターンは異なるし、食事の内容もその場に遭遇しなければわからないことが多い」ということで、まったくその通りだな、と思います(だから、カルテには、いってみれば適当な単位数を記載しているだけです?!)。
 しかし、それでも、異常な高血糖をおこさず、日中も夜中も低血糖は起こさず、朝起きられなかったことなどは全くなく、学校生活や会社生活を満喫しておられます。
基本知識の上に自分の体験を蓄積していく
 先ほどの女性とは、どこが違うのでしょうか。
 1つは、いまの状態よりも、これからのこと(これからの活動量とか、次回の食事はいつごろになるのか、など)に焦点を当てて考え、インスリン注射量を決めて注射していることです。
 2つには、使用している数種のインスリン製剤がどのように自分の血糖を降下させるか、よく理解していることです。自分の場合は○○○・・・のように血糖を下げるのだ、と知っていることです。○○○のようなものを食べるときはだいたい○単位注射すればいい、と知っていることです。朝寝坊したときはどうすればいい、と知っていることです。
 3つには、自分では「○単位くらいかな」と思ってインスリン注射量を変化させてみたがうまくいかなかった、その理由は○○○・・・・だと分析できていることです。
インスリン注射の基本をまずできていること
 このことを先ほどの女性にお話ししたところ、びっくりされるばかり。そんなふうにして血糖をコントロールできるなんて。「血糖が上がったら注射する」これを基本にしていたようです。これまでの主治医もたぶんいろんな話をしていたと思うのですが、耳を貸さなかった。主治医のいうことにしたがっていたら上手くいかないという思いの方が強かったのでしょう。
 ここでの教訓は、基礎的な知識はやはり必要である、ということです。微分積分は知らなくても足し算掛け算は必要、というわけです。
自分に合ったやり方を身につける
 多くの情報があり過ぎると、返って自分が身動きできなくなることにもなりかねないのです。
 たとえば、先ほどのインスリン調節[インスリン製剤の種類はかならずしもアナログインスリンではなく、N(中間型)とR(速効型)を頻回注射している方でも]をどのようにしているか当方にはよくわからないが、HbA1cがいつも良好な20代前半の若者。太っていない。このような患者さんはカーボカウンティングをしていません。ざっくりと食品交換表の「表1」の量に留意しておられますが。もちろん、計算機を持ち歩いてなんかいません。夜中に、もしかして低血糖になっているのでは?と危惧し、通常の生活の中でCGMSをつけてもらいましたが、夜中に低血糖は起こしていませんでした。もちろん、「朝低血糖で起きられなかった、という経験は思い出してみてもないな」というわけです。外来受診時にも、インスリンに縛られているとか、食べることに振り回されているとか、1型糖尿病というモノに縛られているなとか、全くこのような印象を受けないのですね。
 もう一人、なかなかHbA1cが下がらないと19歳時に紹介初診されたすこし太目の20代の女性。先日、HbA1cがとうとう7%を切りました。この方は「SMBGをするのがいや、縛られる感じがしていや」といわれるので、「それならSMBGをしないでやりましょう」と初診から経過して3年。SMBGをしないにもかかわらずだんだん低下してきて、やっとここまで来ました。
 もちろん、早くから良好なHbA1c値にできるのがベストですが。1型糖尿病を受け入れる、インスリン注射を受け入れる、などなど、心の中に葛藤が生じているときに、これがベストの方法だからやりなさい、といわれても、なかなか理性と感情は一致できないことが多く、うまくいきません。
 納得できる、無理のない、自分に合ったやり方を見つけることが、一番よろしいようです。

[追記] 2009年のアメリカ糖尿病学会で
 アメリカ糖尿病学会(ADA)にはいくつか賞があるのですが、今年度のベスト糖尿病教育者賞に、アメリカ・サンジエゴ大学医学部のエデルマン教授が表彰されました。ご自身が小児期発症1型糖尿病でインスリン治療をずっと行っております。とても朗らかな方で教授然としたところがまったくありません。
 表彰の対象は、主に、長年のTCOYD活動(Taking contorol of your diabetes)に対してです。これは、患者さんたちとの、大きな州を超えた糖尿病教室活動ともいうべきものです。
