47 Set Target HbA1c Level for the Next Visit?

We have looked at various indicators for blood glucose control. HbA1c, however, still remains the golden standard.
There is likely to be some change in reference value or normal range of HbA1c that is a threshold value to determine if one has diabetes.
The purpose of the change is to make the value consistent no matter where it is measured, in Japan or in other countries, and also to enable purer HbA1c to be measured.
When you visit a hospital, you have HbA1c measured and receive the results. In this article, I am going to talk about setting target HbA1c for the next visit. There may be some cases where you do not receive HbA1c results in time for seeing your doctor. Ideally, the order should be having blood sampling for HbA1c and seeing a doctor with the results.

Able to guess own HbA1c almost right when seeing a doctor
I do not know how many of you think it is totally impossible to guess HbA1c value right. However, among those who have been patients at our outpatient clinic for years, there are quite a few patients who can guess their HbA1c value almost right.
When a patient asks me, "My HbA1c has gone up, hasn't it?" before I present the results, it has actually gone up in most cases. The patient could guess it right probably because he was reflecting on a number of factors that would lead to higher HbA1c while waiting to see me.
In fact, it is very important and valuable for the patients to reflect on events that would influence HbA1c. These patients already understand the theory as to what makes HbA1c rise.
Meanwhile, if I tell my patient, "Wow, it's great", he would oftentimes say, "As I thought". This implies that he knew his HbA1c had gone down even before it was measured.
This is also impressive because he has already reflected on various factors between the last visit and the visit this time and is putting self-evaluation into practice.
I once had a patient with type 1 diabetes who had been on insulin for nearly thirty years. She told me, "It is not difficult for me to bring my HbA1c to the level you would like". At that time, I was surprised to hear that, but now it all makes sense.

Let's estimate your next HbA1c
Once you can guess your HbA1c almost right, why don't you try to estimate your next HbA1c? You could imagine HbA1c value that your would like to achieve at the next visit.
Of course, there is nothing wrong if you did not guess it right. You could simply lead a daily life while picturing what HbA1c level you could achieve and find out the results when seeing your doctor.
You do not have to share the value you had estimated with your doctor. You can just keep it in your mind.

次回の目標HbA1c値を決めてみる?

 いろいろな血糖コントロール指標の検査項目が出てきましたが、なんといってもいまなおゴールデンスタンダードであり続けるのは、HbA1cです。
 HbA1cの基準値というか、正常域というか、糖尿病であるなしの境の値は、これから少し変わるようです。日本で測定しても、海外で測定しても、その値が同じであるように、またこれまで以上にピュアなHbA1cを測定できるようにするためです。
 今回は、医療機関に受診時に、自分のHbA1cを知ってから、次回のHbA1cをいくつにするか、目標を決めてみよう、という話しです。
 受診時にその時のHbA1c値が、外来診察時までに出てこないというところも、まだあるかもしれませんが、できれば、採血してHbA1c値の結果がでて診察する、という流れでありたいものですね。
診察時のHbA1c値をほぼあてることができる
 こんなこと、できっこない、と思う方が多いのか、少ないのか、わかりませんが、当方の外来に長く通院している方の中には、ほぼ当てることができる方がけっこういます。
 結果を出す前に、「先生、上がっているでしょう?」と先に話かけられるときは、ほとんどHbA1c値は上がっています。たぶん、上がるようなことが多々あった、ということを、待っている間に、思い起こしていたからでしょう。
 しかし、これはとても大切なことで、この方のこの思い起こしている時間はとても貴重な時間ともいえます。こうしたらHbA1cが上がるという理論がもうわかっている、ということになります。
 逆に、「わあーすごいね」と先に感想を述べると、「やっぱりですか?」と、答えられることが多いということは、今日のHbA1cは下がっているな、と測定する前からわかっていた、ということになります。
 これもすばらしいことで、受診時に、前回から今日までをすでに振り返ってしまっていて、そのことに自分で評価している、ということになります。いわゆる自己評価ですね。
 以前ですが、長くインスリン注射している、そのころはほぼ30年近くインスリン注射している1型糖尿病の患者さんが、「先生の希望するHbA1c値にすることは、難しいことではないのだ」と、当方に話してくれたことがありました。そのときは、すごいことを話しされるな、と思ったのですが、いまにして思えば、なるほど、と思うばかりです。
次回のHbA1cをほぼ予測してみましょう
 自分のHbA1cをほぼ当てることができるようでしたら、次回のHbA1cを予測してみませんか。自分は、今度は、いくつのHbA1c値にしてみたいと。
 もちろん、当たらなくてもいいのですね。当たらなくてもけっこうです。いくつのHbA1c値にできるのかな、と思いつつ、生活してみて、受診時にその結果を見る、というながれです。
 もちろん、予測値を、主治医の先生に言う必要はありませんね。自分の心の中にしまっておくだけで、いいですね。
