30 Can you spread your fingers well?

Do you observe your palm carefully? Can you bend and stretch fingers without any pain? Can you form your hand into the shape of a rock, paper and scissors smoothly?
When I see my patients for the first time, I make sure to carefully observe their hands. I ask them to open their palms wide to check if they can stretch their fingers well and if there is no contractural "lump" in their palms.

Cannot extend ring finger
There is well-known contracture diagnosed as "Dupuytren's contracture" with which you develop a lump around the base of ring finger in most cases. Because of this, you cannot extend your ring finger well enough. I learned when I just started as a doctor that I could diagnose a person with such lump as "having diabetes for ten years" in most cases. Although this is a matter of rough speculation, diabetes patients with poor blood glucose control for about ten years seem likely to develop such lump.

Fingers of both hands do not touch each other when holding palms together
There is a known symptom called "restricted joint motion". If you develop this symptom, you cannot make your fingers of both hands touch each other completely when holding your palms together like you do when praying at a shrine. This symptom is also likely to develop in patients having diabetes for a long time.

Other symptoms
Symptoms to which I pay extra attention when seeing my patients include trigger finger, pain at the base of fingers and limited finger mobility. These symptoms can be found very often.
With such symptoms, you will have difficult time typing computer keyboards or cutting flower/branches with scissors in flower arrangement due to severe pain at the base of thumb. You may feel pain even to wash your face and feel gloomy. These are symptoms of tenosynovitis. The muscle around the base of thumb could become thinner, and pain may develop even in the elbow.

Will get better with treatment
When I have patients with these symptoms, I refer them to orthopedic specialists specialized in "surgery of the hand". Then, their conditions get much better close to full recovery.
Such patients are often diagnosed as having tenosynovitis of fingers, carpal tunnel syndrome or cubital tunnel syndrome and so forth.

指がうまく開きますか?

 皆さんは、手のひらを良く見ていますか。指の屈曲は痛みもなく、スムーズに、グー、チョキ、パーができますか。
 私は、初診のときはかならず患者さんの手のひらもしっかり見ます。指が十分に伸びるか、手のひらをしっかり開いてもらって、拘縮した「しこり」がないかなどを診ます。
薬指の伸展がうまくできない
 有名なところでは、「デュプイトレン拘縮」という診断名がついているものがあります。これは、多くは薬指の付け根あたりに、ごりごりのしこりができるものです。そのために、薬指の伸展がうまく十分にできません。このようなしこりがあると、「糖尿病が10年経過している」とほぼ診断できると、医者に成りたてのときに習いました。おおまかな推測の域のハナシです。しかし、血糖コントロールのよろしくない糖尿病が約10年も続くと、このようなしこりができやすいようです。
両手を合わせても指がぴったりあわない
 また、有名なところでは、「関節運動制限」というものがあります。これがあると、神社にお参りするように両手を合わせてもぴたっと全部の指を左右あわせることができません。これも経過の長い糖尿病があるとおこりやすいといわれています。
この他にもこんな症状が
 さらに、ここはどうかとよーく注意して観察するのは、ばね指や、指の付け根の痛みや、指の伸展障害です。とても多く発見します。
 パソコンのキーを叩くのが困難になったり、生け花のときにハサミで花木を切るのにとても親指の付け根がいたくなったりします。顔を洗うのにも痛いなーと感じ、気分がうっとうしくなることがあります。腱鞘炎です。また、親指の付け根のあたりの筋肉がうすっぺらになったり、肘の部位にも痛みがあったりすることがあります。
治療すると良くなります
 整形外科の「手の外科」を専門とする先生に紹介することにしています。とっても良くなります。ほぼ完治するくらいになります。
 診断名としては、各々の指の腱鞘炎、手根管症候群、肘部管症候群、などです。
 最近の新聞に掲載された記事をひとつ紹介しますと、毎日新聞2007年6月29日の「くらしナビ」に、専門の先生が書いておられます。
