26 Can I Take Insulin with Me When Going Abroad?

Many of you may travel abroad during summer.
And you may get worried if you can take insulin with you on board or if you can put insulin in a carry-on luggage.
Since the terrorist attempt at the London's Heathrow Airport on August 10, 2006, airport security at departure has been tightened indeed. This is based on the adoption of regulations recommended by the International Civil Aviation Organization (ICAO) on "liquid restrictions on cabin luggage". In Japan also, the restrictions have been adopted for international flights since March 1, 2007.

Medical certification in English needed?
I will give you the answer first. You can carry insulin aboard without any problem. Some travel agencies may tell you that you will need to have an English description of insulin you are brining. This, however, is not an obligation.
In fact, one of my patients is traveling back and forth between Japan and other countries without an English description of insulin or an English medical certificate for diabetes. Another patient has just come back from a Caribbean cruise. Both of them have not encountered any problem.
There is another patient who travels back and forth between the U.S., Germany, and Japan on business. He has had no problem, either.

Liquids that cannot be easily identified
Liquids that would be questioned are those other than medications, baby milk/food, special restricted diet and so forth. They are, for example, drinking water in plastic bottles, bottled skin lotion and cleaning solution for contact lenses.
If you would like to carry these liquids onto the plane, you just need to put them in containers of 100mL or less, put them in a clear plastic bag (the size of a freezer bag will be the best) and present them to officials at security checkpoints.

Take insulin, syringe and blood glucose meter in a carry-on
You could put extra insulin needles in your check-in luggage, but you should take insulin, needles you need during your flight and blood glucose meter with you on board. You could just take them as a carry-on and have them x-rayed at the security check.

Baggage inspection varies from country to country
Baggage inspection is done thoroughly in developed countries such as the UK and the U.S.
In these countries, if you have a computer in your bag, you need to take it out and have it x-rayed. If you are wearing a jacket and/or a belt, you will be asked to remove them for security screening, and even shoes. Even if you do not understand English, you do not have to worry as officials at security checkpoints will give you instructions with gestures.
In the summer of 2007, I went to Ghana in Africa to attend an academic conference and flew back to Japan after stopping at Dubai for transit. There was no such inspection as I explained above in neither country.
I also went to Beijing at the end of May in the same year. The security screening there was unusual. The officials bothered to hand me a clear plastic bag which I had to use for security check.
Security check does vary between countries.

Don't forget to drink enough fluids while on a plane
When you hear about all of these liquid restrictions, you may wonder if you have to make do without water. That is not the case. Once you go through security checkpoints and move to areas where duty free and other shops are, you can buy beverages in plastic bottles and take them with you on board.
Also, cabin attendants oftentimes give you bottled water after in-flight meals. This is because it is very important to take fluids to prevent economy class syndrome, blood clots caused by staying in the same position for a long time and dehydration. And please don't forget to rotate your ankles frequently.
The air is very dry in a plane. I always make sure to take plenty of fluids while on a plane.

海外へのインスリン製剤持ちだしは大丈夫？

　夏は海外に出かける方も多いことでしょう。
　海外にでかける時は「インスリンは大丈夫か？」、インスリン製剤を機内に持ち込めるか、荷物の中にいれてよいか？と不安になられることと思います。
　平成18年8月10日の英国ヒュースロー空港でおきたテロ事件以来、たしかに出国の際の警備はきびしくなりました。これは、国際線民間航空機関（ICAO）による「国際線航空機内への液体物持ち込み制限」の導入という規制によるものです。わが国でも、平成19年3月1日からこの規制を国際線に導入されています。 　結論からいいます。インスリン製剤の機内への持ち込みはまったく大丈夫です。それから、旅行会社によってはインスリン製剤の中身を英文で書いたものがほしい、というところがあります。しかしながら、これは義務ではありません。
　実際に、インスリン製剤や糖尿病であることの英文証明書なしで、私の患者さんは海外を行き来しています。先日はカリブ海のクルーズからもどってきた方もおられます。問題なしです。
　また、ある方はお仕事で、アメリカ、ドイツをいったり来たりしておられますが、まったく問題なしです。 　それでは何が問題なのかと言いますと、医薬品、ベビーミルク／ベビーフード、特別な制限食など以外の、液体物です。つまり、ペットボトルの飲料水、化粧水の瓶、コンタクトレンズの洗浄液など、などです。
　これらを機内に持ち込みたい時は、100mL以下の容器にいれて、透明なプラスチックバック（フリージングバックくらいの大きさがもっともいい）にいれて、手荷物検査の時に係りの方に見せればいいのです。 　インスリン針の余分は預ける荷物の中にいれてもいいですが、インスリン製剤、フライト中の必要な針、血糖自己測定器は手荷物として飛行機に乗りましょう。普通に手荷物検査のエックス線検査を通してください。

　手荷物検査は英国、アメリカなど先進国ではきちっとおこなわれています。
　なにしろ、これらの国では、パーソナルコンピュータもカバンから出してエックス線検査を通す必要があるし、男の方はジャケットを脱ぐことも必要になるし、ベルトを取ることも要求されます。また履物も脱いで検査に出すことも要求されます。　でも、これらは英語が理解できなくても、係官からジェスチャーで指示されるので、心配はいりません。
　2007年の夏に、学会参加のため、アフリカのガーナに出かけました。ドバイで飛行機を乗り継いで帰国しましたが、いずれの国でもまったくこのような検査はありませんでした。
　 　2007年の5月末に北京に行きましたが、この地はまた変わっていて、透明なプラスチックバックをわざわざ渡されます。渡された透明なプラスチックバックを使用しないといけないのです。
　国によって、いろいろだなと思いました。 　それでは、水分を取ることなく、がまんしなければならないのか、と思われるでしょう。大丈夫です。この検査を通過すれば、つまり免税品を販売している場所などでは、ペットボトルの飲料水を購入することもできますし、そのまま機内に持ち込むこともできます。
　また、機内では食事のあとにペットボトルの飲料水を配布されることも多いのですね。これはエコノミークラス症候群、つまり座りっぱなしと脱水による血栓症を予防するために、水分を摂取することがとても大事なこととされているからです。そして、足首などをこまめに回しておきましょう。
　機内はとても乾燥しています。私も水分だけは十分に取るようにしています。
